Los Angeles Healthcare & Medical Office Construction

LA is one of the largest healthcare markets in the country. TCG builds medical office buildings, clinics, imaging suites, and ambulatory surgery centers with the FGI compliance, med gas, shielding, and infection control that pass inspection, under one design-build contract.

Building Healthcare in a Major Medical Market

Los Angeles is anchored by world-class health systems including Cedars-Sinai, UCLA Health, USC Keck, and Kaiser Permanente, and the demand for outpatient space keeps growing as care shifts out of hospitals. Medical office buildings, clinics, urgent care, imaging centers, and ambulatory surgery centers all need a contractor who understands healthcare code, not just commercial buildout. TCG builds it as an integrated commercial general contractor from our Venice, CA office with in-house architecture and MEP engineering.

The key distinction in California is jurisdiction. Licensed hospitals and skilled nursing fall under HCAI (formerly OSHPD) with its own plan review and strict seismic standards, while medical office buildings and most clinics go through LADBS. Knowing which path your facility takes, and designing to it from day one, is the difference between a smooth project and a stalled one. TCG scopes that early as part of design-build.

How We Build It

FGI Guidelines, Med Gas, Shielding, and Infection Control

FGI Guidelines drive the layout. Exam rooms, procedure rooms, and operatories carry minimum clearances, mandatory hand-hygiene placement, and specific air-change and pressure-relationship requirements. Our MEP engineers design the HVAC to hit those pressure relationships and filtration levels, because a medical final inspection checks them directly. Get them wrong and the space does not open.

Specialized systems are the technical heart. Medical gas (oxygen, vacuum, medical air) requires brazed, certified, and third-party-verified piping. Imaging suites for X-ray, CT, and MRI need lead or other radiation shielding designed by a physicist, and MRI adds RF shielding and strict ferromagnetic control. TCG coordinates these specialty trades and runs equipment procurement so the imaging or surgical equipment, the room, and the utilities all line up.

Infection control during construction. Most LA medical work is tenant improvement inside an occupied building, which means an ICRA (Infection Control Risk Assessment): negative-air containment, HEPA filtration, sealed barriers, and dust control so construction never threatens patients next door. California seismic (SDC D/E) requires bracing of equipment, ceilings, and med gas. We self-perform the seamless, cleanable flooring healthcare demands and coordinate the whole job under one contract.

LA Cost Guide

How Much Does Medical Construction Cost in Los Angeles?

In the LA metro, medical office buildings run roughly $260 to $430/SF and specialty medical (imaging, surgery, procedural) $380 to $620/SF. Medical tenant improvement runs roughly $250 to $620/SF depending on whether the space already has medical infrastructure. LA's 1.15 to 1.40x cost multiplier applies on top, driven by prevailing wage, seismic, and Title 24.

Cost concentrates in mechanical (the air-change and pressure requirements), medical gas, shielding, and specialty equipment, not the shell. An HCAI-licensed facility costs more than a comparably sized medical office building because of the heightened review and seismic standards. See the medical office cost guide and urgent care cost guide.

Healthcare Construction in LA: FAQ

Common questions about building healthcare and medical office facilities in the LA metro. See our full FAQ page for 70+ answers.

In the LA metro, medical office buildings run roughly $260 to $430/SF and specialty medical (imaging, surgery) $380 to $620/SF, with medical TI around $250 to $620/SF. LA carries a 1.15 to 1.40x multiplier. See our medical office cost guide or run the estimator.

Licensed hospitals and skilled nursing facilities fall under HCAI (formerly OSHPD), which has its own plan review and strict seismic standards. Medical office buildings and most clinics go through LADBS. TCG identifies the correct path early and designs to it as part of design-build.

Yes. Medical gas requires brazed, certified, third-party-verified piping, and imaging suites need radiation shielding designed by a physicist, with MRI adding RF shielding and ferromagnetic control. TCG coordinates these specialty trades and runs equipment procurement so the room, utilities, and equipment align.

FGI Guidelines set minimum clearances, hand-hygiene placement, and air-change and pressure-relationship requirements for exam, procedure, and operating rooms. Our MEP engineers design HVAC to hit those pressure relationships and filtration levels, which are checked directly at medical final inspection.

Yes. Occupied medical TI requires an Infection Control Risk Assessment (ICRA): negative-air containment, HEPA filtration, sealed barriers, and dust control so construction never threatens patients nearby. TCG builds to ICRA protocols as a standard part of healthcare work.

LA sits in Seismic Design Category D or E, so medical equipment, ceilings, med gas lines, and imaging equipment all require seismic bracing, and HCAI-licensed facilities carry heightened seismic standards. Our structural engineers design the bracing and anchorage as one system.

Medical office TI typically runs 5 to 10 months plus LADBS permitting, while HCAI-licensed facilities take longer due to added review. Design-build compresses the schedule by 15 to 30 percent. A second-generation medical space is faster than a cold shell.

Both. TCG provides in-house architectural design, MEP engineering, and structural engineering tuned for healthcare, and also builds from owner-provided plans as a general contractor. Start with the estimator.
